Before Your First Appointment
As a new patient to our Foundation, we request that you arrive twenty minutes before your first appointment. You will be asked to complete paperwork necessary for treatment and billing. You may also be requested to complete a medical history questionnaire.
The registration desk is on the third floor of the main (Soquel Avenue) clinic. You can register a new patient, update your insurance information and make any payments.
If you are visiting any of our other locations, the receptionists will take care of your registration and payment activities.
When you come to the registration desk, please bring the following items:
- Current insurance cards
- HMO or insurance co-payment if applicable
- Patients without insurance are required to pay a deposit. The deposit amount will vary depending upon department and anticipated services.
In addition to these items, do not forget to bring items requested by the physician's office when you made your appointment.
